  1. CONTINUOUS IMPROVEMENT – We have consolidated questions and continue to tweak definitions and wording of the questions to make this more user friendly.
  2. We have made every effort to request data that is customarily maintained. You are generally not required to research your answers. We encourage you to use your BEST ESTIMATE versus spending valuable time gathering data that is not formally maintained.
  3. EDIT CHECKS ARE BUILT IN (the on-line version). The survey will prompt you to verify data that does not meet our expectations. You have the option of providing explanations for the discrepancy or to change the information. Some data, such as totals, must be completed accurately before you can submit your data. Also, when in doubt USE WHOLE NUMBERS even when percentages are requested.
  4. The survey is SEGREGATED BY FUNCTION to allow you to delegate the collection of the data.
  5. ON-LINE CAPABILITY, with owner issued passwords allows you to share the data collection responsibilities while maintaining confidentiality of the individual sections. Only the owner has access to the complete questionnaire. On-line submission allows companies to PARTICIPATE ANONYMOUSLY using a Hotmail or Yahoo email account.
  6. This survey should be prepared for each COMPLETE FINANCIAL REPORTING UNIT, meaning balance sheet and profit and loss responsibilities. For multiple locations you may use an average answer for all locations.
  7. Use your LAST FISCAL YEAR RESULTS in answering the survey questions, unless otherwise specified.
  8. This survey may include Canadian, Mexican, or US processors. Use your local currency – BE CONSISTENT.
  9. OPTIONAL LIMITED FINANCIAL PARTICIPATION – Companies that are reluctant to share financial data need only supply basic sales, material and labor data and be able to receive the survey report. However, Companies choosing to participate with limited financial data will also only receive limited financial data in the final report.
